The Stamford VA Primary Care Clinic provides general internal medical care. This includes care for immediate and chronic health concerns, preventive medical care, physical exams, diagnostic laboratory testing, general X-rays and limited immediate pharmacy services.

ADVANTAGES

Veterans will have one primary care provider to coordinate care on an ongoing basis.
Veterans can receive primary medical care closer to home.
Veterans will still be eligible for specialty outpatient care, inpatient care and all other entitlements through the West Haven or Newington Campuses.

Please mail a completed application along with a copy of your DD-214 and discharge papers to:

DIRECTIONS:

From New York

Via Interstate 95 - Take Exit 7 (Greenwich Avenue) toward CT-137 North. Turn slight left onot State Street (South).  Turn left onto Washington Boulevard. Take a slight right onto 2nd Street onto Summer Street.

Via Merritt Parkway - Take Exit 34 (Long Ridge Road).  Take a right at the end of the ramp.  Follow Long Ridge Road which will turn into Summer Street.  Clinic located in the "Health Sciences Center."  Parking available under the building.

From New Haven

Via Interstate 95 - Take Exit 8 (Elm Street). Turn right onto Elm Street.  Elm Street becomes Grove Street.  Turn left onto Hoyt Street.  Turn right onto Bedford Street.  Take a left onto 2nd Street. Turn left onto Summer Street.

Via Merritt Parkway - Take CT-15 South toward New York City.  Merge onto CT-8 South via Exit 52 toward Bridgeport.  Merge onto Interstate 95 toward New York City.  Take Exit 8 (Elm Street).  Turn right onto Elm Street. Elm Street becomes Grove Street.  Turn left onto Hoyt Street.  Turn right onto Bedford Street.  Take a left onto 2nd Street.  Turn left onto Summer Street.
