The Stamford VA Primary Care Clinic provides general internal medical care. This includes care for immediate and chronic health concerns, preventive medical care, physical exams, diagnostic laboratory testing, general X-rays and limited immediate pharmacy services.

ADVANTAGES

Veterans will have one primary care provider to coordinate care on an ongoing basis.
Veterans can receive primary medical care closer to home.
Veterans will still be eligible for specialty outpatient care, inpatient care and all other entitlements through the West Haven or Newington Campuses.

Please mail a completed application along with a copy of your DD-214 and discharge papers to:

DIRECTIONS:

Traveling south on I-95:Drive south on I-95 toward New York City. Take Exit 9 (US-1/State Hwy 106) toward Glenbrook. Turn right at East Main Street/US-1 (signs for Glenbrook). Continue to follow US-1. Turn right at CT-137/Washington Blvd. Turn right at 2nd Street/Second Street. Turn right at Summer Street. Clinic will be on your left.

From NYC and traveling north on I-95: : Take I-95 north toward New Haven. Take Exit 7 for Greenwich Avenue toward State Hwy 137 North. Merge onto McCullough Stree/South State Street. Turn left at CT-137/Washington Blvd. Turn right at 2nd Street. Turn right at Summer Street. Clinic will be on your left.

From Hartford and points north: Take I-91 South to Exit 17 (E Main St/State Hwy 15 S) toward the Wilbur Cross Parkway. Keep left at the fork to continue toward CT-15 South and merge onto CT-15 South. Take Exit 35 toward Stamford. Merge onto CT-137/High Ridge Road. Continue to follow High Ridge Road (about 2.8 miles) which will turn into Summer Street. Continue onto Summer Street. Clinic will be on your left.
