The Windham VA Primary Care Clinic provides general internal medical care. This includes care for immediate and chronic health concerns, preventive medical care, physical exams, diagnostic laboratory testing, general X-rays and limited immediate pharmacy services.

ADVANTAGES

Veterans will have one primary care provider to coordinate care on an ongoing basis.
Veterans can receive primary medical care closer to home.
Veterans will still be eligible for specialty outpatient care, inpatient care and all other entitlements through the West Haven or Newington Campuses.

HOW TO ENROLL
Please mail a completed application along with a copy of your DD-214 and discharge papers to:

DIRECTIONS:
From Hartford: Take I-84 East to Exit 59 to Route 384 East. At end of expressway continue on to Route 6 East to Willimantic. At Junction of Routes 6 and 66, take left onto expressway (Route 6) to second exit (not numbered). Take right at end of ramp onto High Street. At first four-way stop, take right onto Prospect Street to end. At stop sign turn left and make an immediate right to the clinic.

From New Haven: Take I-91 North to I-84 East. Follow directions above.

From Old Saybrook: Take I-95 North to Route 396 North to Exit 82 West into Norwich onto Route 32 North to Willimantic which will run into Main Street. Take a right at Mansfield Avenue, which is opposite a KFC restaurant. Continue past stop sign up the hill to the second hospital sign on the left. Follow signs for parking and main entrance to hospital.

From Putnam: Follow I-395 South to Danielson to Route 6 West to Willimantic where Route 6 becomes an expressway. Take left at first stop sign onto High Street. Take right at first four-way stop sign onto Prospect Street. Continue to end. At stop sign, turn left and make an immediate right to the clinic.
