Requesting Medical Records

As a reminder, the Release of Information (ROI) office has several options for veterans to request copies of your medical records without physically coming to the facility. 

  • VA Form10-5345 for third party requests. 

Completed request/authorization forms can be sent to Release of Information:

  • Fax to 860-667-6716 (Newington) or 203-937-3461 (West Haven)
  • Via My HealtheVet secure messaging
  • Mailed to ROI at 555 Willard Avenue, Newington, CT 06111 or ROI 950 Campbell Avenue, West Haven, CT 06516 

Please note that some requests can take up to 20 business days to process. Records will be mailed directly to the veteran or third-party address listed on the request/authorization form. 

Records can be accessed directly via My HealtheVet. You can also visit  https://www.va.gov/health-care/get-medical-records/ 

Veterans without Internet access who need a form mailed to them should contact the ROI office at 203-932-5711 ext. 3229 (West Haven) or 860-666-6951 ext. 6414 (Newington).
