Care/Case Management and Telehealth
The Case Management & Telehealth program is based on an integrated, collaborative case management model utilizing an interdisciplinary team approach. Telehealth modalities include Home Telehealth (HT), Clinical Store & Forward (CSF), and Clinical Video Telehealth (CVT). The aim of the program is to provide proactive, high quality, timely care with a focus on health management.
Case managers are specially trained registered nurses and social workers responsible for care coordination across the continuum of care for patients identified as high risk, high cost, or at high risk for decline. Case managers coordinate services as appropriate with VA and non-VA providers, Patient Care Coordinators (PCC), Surgical Case Coordinators (SCC), Telehealth Care Coordinators (TCC) to support Home Telehealth (HT) program, and the interdisciplinary team as appropriate to provide the right care, at the right time, in the right place, at the right cost, each and every time. Telehealth Clinical Technicians (TCT) support all telehealth services (home telehealth, store and forward telehealth, and clinical video telehealth) at both campuses and all community based outpatient clinics.
