Nursing care is provided in Acute Medicine, Surgery, Intensive Care Units, Psychiatry, Ambulatory Procedure Unit, Ambulatory Treatment Units, Sub-Acute/Extended Care/Palliative Care/Respite Care, Medical Emergency Room, Psychiatric Emergency Room, Ambulatory Care, Primary Care Firms, Community Health Care, and special programs including Dialysis, Blind Rehabilitation, Case Management and Telehealth, Home Based Primary Care, Spinal Cord Injury, ALS and the Clinical Call Center.
All nursing care is planned and supervised by a registered nurse. Reassessments are completed by registered nurses at defined points of care and whenever indicated by patient condition. Patient care is planned, coordinated and delivered through inter-professional collaboration in all inpatient care settings.
In ambulatory care, the registered nurse assumes the pivotal role of Care Manager working directly with patients, family members, and the inter-professional team to implement patient centered care based on individualized assessment to prevent disease and manage chronic illness throughout the lifespan. The Registered nurse in the outpatient setting is engaged in active inter-professional learning to improve collaboration and communication within the inter-professional team that results in improved health outcomes to the veterans.
